For your convenience, we’ve partnered with TheraOffice to allow you to fill out your medical forms online. To help expedite your first visit, please register below and fill out the forms that pertain to you prior to your visit. 

 

If you choose to fill out the patient form packet at our office, please arrive 20-30 minutes before your appointment to allow enough time to get them completed before your appointment.

 

Please bring a list of your prescription medications, over the counter medications, and supplements, on this list please include frequency of intake as well as the method of administration for each drug.

We ask that you please wear comfortable clothes with which to exercise in as well as clothes that will allow easy access to the area of treatment.

At California Therapy Solutions, we accept most major PPO insurances and will bill your carrier on your behalf. We also accept Medicare and will work with all workers' compensation insurance carriers.
