HOAG PARTNER INFORMATION

Dear St. Joseph’s/Hoag Affiliated/Hoag Medical Patient:

 

We value you as a patient and thank you for choosing California Therapy Solutions for your physical therapy care!

 

With the new relationship between Hoag Hospital and St. Joseph’s Heritage Health, we want to be sure that we, at CTS, can see you for your physical therapy needs.

 

Effective January 1, 2021 the IPA will change to Hoag Partners. If you wish to keep your Hoag healthcare providers, including California Therapy Solutions, you will need to change your group association on your insurance plan.

 

Please check your group affiliation during your open enrollment period. Your human resources/benefits coordinator, at your place of employment, will be able to assist you with this matter. Instead of continuing with St. Joseph’s please tell your insurance carrier you wish to enroll with Hoag Affiliated/Hoag Medical under Hoag Partners IPA.

 

If you are a Medicare HMO patient you will need to change your group affiliation during Medicare’s open enrollment period beginning October 15, 2020. Your Medicare open enrollment period ends December 7, 2020 for all changes taking effect January 1, 2021. Please call the telephone number on the back of your insurance card to make any changes to your plan.
